The hunger/thirst mechanic makes the settlement system work very well. You see a slightly intact house in the wasteland, and the first thing you want to do is find out if it has a bed in it. It gives the entire experience a Dark Souls feel, but instead of having single bonfires placed along the gameplay path, it requires you to search high and low throughout the entire world for beds. Only being allowed to save at beds is the most compelling aspect. I think this survival mode is even better than the excellent one in New Vegas. I heard there was a new survival mode that was similar to New Vegas, so I started a new game using it. The Pro patch came out, and I loaded it up to check it out. I ended up getting too bored to continue. I played through about 1/4 of Fallout 4 at release.
